[Clinical] Is there any value in palpating/percussing the heart?

So I'm in my first year of medical propedeutics, second year of med school, studying the cardiovascular physical examination. The textbook tells me to palpate the ictus cordis/apex beat and define its location and diameter; palpate the right ventricle at the superior left sternal border; palpate for pulmonary artery aneurysm or feel for the aorta in systemic hypertension.

The book goes on to say if we're not able to palpate the ictus cordis and the right ventricle, we can percuss the anterior thorax to estimate the size and location of the heart.

Now, I can see the value of auscultating the heart valves for murmurs/clicks etc., albeit I have to concentrate real hard to even hear anything. The clinical and pathophysiological correlation is pretty straightforward.

But I cannot for the love of god palpate the ictus cordis or the right ventricle or the great vessels or whatever, let alone determine the diameter of the apex beat. And I absolutely cannot tell the difference between the percussion of the precordium or the lung. It sounds exactly the same whether I'm percussing the lateral wall of the thorax or the left sternal border. Note that I find it really easy to tell the difference between percussing the gastric fundus/liver/lungs.

The entire chapter of the cardiovascular physical exam basically tells me to find the jugular venous pressure (which I CANNOT find), palpate then auscultate the heart, nothing else. The section of the book on lungs was so much better, with higher yield techniques.

Can you give me some tips for these?

Canine Overload

I wake up most mornings to barking. As I get out of bed, start the coffee, and sit down to my breakfast, I do so to the tune of barking more mornings than not. As I go about my day, three neighborhood dogs take turns. One is particularly shrill and sounds as if it has swallowed a squeaky toy and each bark is being beaten out of it. Another has a bark that is harsh and percussive, and carries for blocks and blocks. The other is a Jack Russell that barks with a frantic urgency you might expect if it were in serious and immediate danger; it's looking for attention from its owner, who ignores this demonstration entirely even as he sits right in front of it.

Rare is the day where there is a prolonged, peaceful period without the noise pollution that is incessant barking. I'm not talking about a "woof woof" indicating that it's time to go back inside, or a few yaps at a pedestrian, but actual hours of repetitive noise, day after day, from multiple sources, left entirely unchecked. This daily auditory assault has my already frayed nervous system in a state of constant alert preventing me from ever fully relaxing.

It has turned me into a snappy, edgy, easily annoyed ball of stress. It has reached a point where even pictures of dogs or the WORD dog is beginning to evoke a similar stress response. And there's no escape.

Commercial for buying a home: Dog in the yard.

Commercial for truck: Dog bouncing around the vehicle.

Commercial for cat litter: Dog in the background.

Commercial for a biologic medication for psoriasis: Human frolicking with their dog.

Commercial for paint: Dog running around the house as people paint.

Then there's social media.

Facebook tabby cat group: "I know this isn't a cat, but here's my f%cking dog!"

Instagram cat accounts: "I also have this dog, which I will feature in every photo of my cat."

Instagram runner accounts: "I like to run with my dog."

Instagram yoga accounts: "My dog likes to climb on me while I do yoga."

Twitter: Dog mom! Be sure to follow my dog's account, too! (Your dog eats its own sh%t, and even if it COULD tweet, it would be about eating more of its own sh%t.)

All Taylor Swift Albums Ranked (2006-2020)

And now we've reached the beginning of the end of CountryMusicStuff's first appreciation week, I loved every bit of this process, even when I was stressed out with deadlines, I kept pushing forward for you guys as a community, this has been the most fun I've had with the sub in a long time and I can't wait to explore other big and underappreciated artists. After 8 albums its time to answer the big one, which album stands tall above the rest, I'd love to know your opinion, even though I'm not touching Taylor Swift for a long time after this, I'll give a little shoutout here and there when I see it fitting. A lot of people this week have come together to appreciate all the music she's put out, and I don't want it to fade away. I'm gonna announce the placements Drum Corps style because this has built up a lot of momentum over the past week, also this was not easy, a lot of these albums were highly competitive, some rose in placement, some did not, a majority of the scores were really tight, and I'm being as critical as can be, because the fanboy in me would put all these albums around 8 or 9 out of 10.

In 8th Place With A Score Of 6.9/10:

8. Lover (2019)

Lover is last for me, as I feel like it has a lot to offer, but it fails to be cohesive, the sound ranges from candy sounding pop, to 80's synth style, to the style of Reputation and country, it's a mess in terms of lyrics and production. There a few instances where we get the peak of Taylor's vocal performance and writing skills, but mostly it feels contrived and lazy. Also this isn't last for my disdain of the trash that is the man, the album score is the average of my song ratings, so The Man only counts for 5.5% of the score. 1 song doesn't determine where I place a record. I've seen records where I give one song a 1/10 and the rest are 8,9 or 10 out of 10, and I give it a high score. The writing and sound for a lot of the record, is kind of dated, to be honest, songs like ME! or You Need To Calm Down's sound was already done 2016 and 2017, and in 2019, the sound of music changed drastically, either trap songs by DaBaby, or mellow pop songs by Billie Eilish and Post Malone trended, I'm not saying it failed as it has sold 4 million worldwide which in the streaming era is huge, and it still landed to be the most popular album of 2019, but chart wise this album didn't land, for an artist a few years back was a head of the times it feels on this record she's playing catch up.

HEAVY METAL INCOMING! rang off of every flat surface, roared from every speaker.

Every Lanaktallan on the planet jerked in shock, some dropping paddy or cud, others urinating in surprise.

The surprise only lasted till the roar was repeated four times more. A Lanaktallan in the Most High Command Complex hit the planetary alert button as it roared out again. The signal went out to the entire planet's defenders.

Red Alert.

Mo'orbys heard the klaxon wailing and galloped for the armory. More and more Lanaktallan surrounded him, all of them galloping for the same complex where the weapons, armor, and equipment was stored. By the time he reached it and found his place in lines thousands of Lanaktallan were waiting to get their equipment. The Company and Battalion Most Highs moved up and down the ranks, making sure everyone was in their positions and handing out stimpacks. Tabbing the stim calmed Mo'orbys down from the anxiety he had felt each time that the Terran battlecry sounded out.

At one point his datalink pinged and he checked it.

Once he drew his armor and weapons he was to report to the airfield. An armored hovercraft personnel carrier would take him to the perimeter of a lithium salt field extraction point where he was to dig in and help Sixtieth Infantry Division hold the extraction site.

You know how your mom never, ever, ever stops talking about how painful and difficult your birth was? How each time she talks about it, it gets longer and more painful and more detailed and more awful? And how that long and difficult experience makes you indebted to her forever? And how when she tells that tale at Thanksgiving every year with three-too-many Chardonnays in her you feel the bile and anger rise?

What, just me? Goddammit, Freud, you are ruining my sex life.

This week’s band went through a truly difficult and painful genesis. They strove for success and fought themselves all the way. The band peaked just as they were coming apart, and the breakup was ugly. There were drugs (a lot more than six). Worse, there was heroin. And no, not the Wonder Woman kind, that’s spelled differently. There was alcohol. There were binges and mistakes. There was a car crash. Then there was another one. There was a feud with Liam Gallagher. OK, that shit is totally understandable. Wanker. There were brushes with greatness. Steve Fisk, Chris Cornell, and our ginger savior all played a part.

Birth is a difficult process. I should know, because I gave birth to a kidney stone once, and I am never letting that fucker forget it.

Today we will take a dive into a painful experience for everyone involved, full of regret and mistakes. But that suffering produced some amazing music, was the bridge between Kyuss and QotSA, and introduced the world to the ashtray-eating, heroin-shooting gargoyle himself, Mark Lanegan.

Painkiller Help

Hey All,

My next checkup with my Neuro is next Tuesday, and I want to ask for some sort of painkiller to help with the pain i'm experiencing in my legs.

Its kind of a deep throbbing pain, not unlike what "growing pains" felt like when i was younger. But it lasts all day and is completely exhausting.

I'm looking for a painkiller that I can take during the day that will allow me get on with my job.

I've been taking nuerofen plus for the pain until now (12.5mg codeine & 200mg ibuprofen), they're over the counter. But i'm finding on certain days that its just not enough, and I don't want to be eating them like candy. Plus too many and i'm groggy AF.

I'm already taking 125mg of Lyrica twice a day for my neurogenic pain, but this is a totally different type of pain and that painkiller does nothing for this leg pain.

So... any suggestions? Any idea if there's something out there that could help?

[shitpost] Nurse Practitioners make practicing Medicine look easy..because they never actually studied it. Personal Experience Comparing the same exact physical Exam MD AND DO vs NP

For My school I have to get an annual physical "each year" (just in case NPs are reading this). For the past 3 years year 1 a DO in my home town did it, year two MD in miami did it, year three...a shady doctor who clearly advertised that he is an MD, gives me an NP instead, which is B.S. (fun fact a bachelor of science take longer to complete than an NP degree).

D.O. Detailed physical asked about my medical history, actually showed compassion. Love the guy, great to always chat with him.

M.D. (first year resident at UM) Elicited from me painfully long history and ordered relevant tests as per USPSTF guidlines. Was impressed about how much he cared.

N.P. 4 minute shitshow for everything, she kept confusing my hep b vaccinations and called them titers, shes like "I can't sign off on this because I need to know if your immune" , and I said school is not asking for titers they want to confirm my past vaccination history. She like but how do i know you immune, and im like they already have my titers, this is a vaccination history not titer check.

  1. " I get this exam annually"-ME ...her response " so when was the last time you got this exam done?"
  2. She checked breath sounds, then straight to abdominal percussion, because why not?
  3. Ordered routine blood work?? wtf nothing in my guidelines says I should get blood work at my age, nothing.

I am tempted to throw the misleading MD under the bus, that advertised in his practice that its an MD run office, but I never actually got examined by an MD.

Edit: Apparently I need to have receipts to back up the fact that you do not need a B.S. to become a Nurse Practioner, here is the link and direct quote from nurse.org :

A couple of weeks ago, I read ‘Did I Scare You? How MJ altered the sensations of racism’. It’s a retrospective review of "GHOSTS" in which Michael's response to his critics is analysed. It’s a great read and very well informed, though I some of the comments it made I wasn’t so sure of. The review focused on Michael’s changing face, how he used it for art and it’s connections between Michael and racism. I don’t disagree with the article but I wanted to articulate for myself what I feel about the movie and put it out there.

GHOSTS is my favourite short film. Combined with HIStory: Past, Present & Future Book I, they are a set of artistic works which has been the most autobiographical of Michael's work. I wanted to share everything I know about it with links to behind the scenes, articles, music and information in one place. I wrote this to post on r/MichaelJackson, I’m not a professional writer or music critic, I’m just a fan so forgive me if my style is a bit messy. To my knowledge, Ghosts isn’t available on any streaming network and there’s no high-quality versions of Ghosts available online but this is the best I can find on YouTube, The sound is off and the picture is stretched from 4:3 to 16:9. Halloween is coming up soon and naturally we think about Thriller, Ghosts and few other tracks at this time of year. I can only hope the estate will at some point scan it to 4K and release it properly.

I can’t just write about Ghosts, “Is It Scary” and “2 Bad” are part of its DNA. Hell, the whole HIStory album should be included given what “Ghosts” is about, even “Morphine” could be included but this post is a biggie, so I won’t. The story of the feature film: “Michael Jackson’s Ghosts” is a far cry from the title of the movie and the track it lends its name from.

Concerta to Vyvanse, first impressions

TL;DR and weekly update at end. 28yr/m for reference

I created a post in here not too long ago defending concerta, explaining how it changed my life and how i mitigated the side effects. Despite this, the inconsistency of the generics had me wondering about vyvanse.

This will be an ongoing observation thread which i will update periodically.

First two days:

Vyvanse (30mg, name brand) compared to Concerta (36mg, generic):

Pros:

  1. Much more consistent, smoother extended release. Also lasts 2-3hrs longer.
  2. No twitchy feelings like with concerta
  3. Slight zombie feeling seems to have gone away, i feel like myself again.
  4. No headache
  5. Not as much dehydration
  6. No anxiety (which may be because my body is already used to elevated heart rate and stimulants after a month on concerta)

Cons:

  1. Not as laser focused. Distractibility: 9/10 w/o meds, 1/10 on concerta, 3.5/10 on vyvanse
  2. More expensive
  3. More hot flashes/sweating
  4. Takes 1-2hrs to kick in
  5. You have to avoid certain foods, drinks & vitamin C

Interesting Note: You don't realize how strong vyvanse is until you socialize or listen to music IMO. Just sitting in a chair working it doesn't feel quite as strong as concerta.

First day taking it: I was working on 6 hours sleep and was exhausted until it kicked in. Once it kicked in i re-sorted all of my files at work into categorized folders. When i got home from work i moved my home office (including huge 4 monitor gaming setup) into another room then cleaned up after. Looked up and it was 10pm. Hard to believe i had no temptations to take a nap despite my fatigue.

Conclusion: This is just two days in and we all know it takes longer to get a good impression how a medication is working for you. It feels so much smoother than concerta in a lot of ways, but the fact i seem to be more easily distracted with vyvanse (like writing this post instead of working) so far may be a deal breaker... time will tell and i will keep you all updated!

Update 1 - One week in:

Strength: This stuff is no joke. I am not able to take naps on this stuff like i could with concerta. It is strong, especially the first 6-7 hours.

Focus/distractability: Is actually pretty good and not as bad as i first thought. However, i do notice i am more inclined to use that focus on things that aren't healthy/work related. I have to remind myself to focus on the right things. If i can focus on the right things i can dish out

Magnus was standing on the bridge of a ship that had been destroyed nearly 300 years ago, staring down the captain as he handed Magnus the codes for the device that would win them the war and send Magnus into suspension for centuries. The captains mouth moved but there was no noise, in fact Magnus couldn’t hear anything. The engines were silent, there was no vibration from the reactor, no rattle through the deck as the weapons fired. His body moved on his own, reaching out for the pass.

Unlike when it had happened for real the pass was hot, searing hot, the symbolism was obvious. He held the burning hot piece of plastic and metal, slipping it into a pocket as he turned to leave the bridge. The blast doors opened into the hall, grey metal and flashing red lights. As he stepped through it changed, the cold metal giving way to an open field of red grass.

*did you think you could hide, oh child mine? *

The voice was warm, like a summer breeze whispering across your back. Sweet as honey, promising sin. A voice that had never frowned, never felt anger, or disappointment. Magnus felt it sweep into his soul, calming him, he wanted to stay here with the voice.

*did you think that I would not find you? That you could escape me? A father knows his child. Will you face the judgment that you RAN from? Will you stand tall, as I read you the sins of your fathers, will you accept the punishment. *

Magnus sat down, running his hands through the grass. He wanted to stay here, with the voice. The words meant nothing, not when the voice was so kind.


Magnus woke with a start, adrenaline flooding his body as the medical pod opened above him. he had been dreaming, he remembered being warm, so why did he wake up in a panic? There was a pair of rangers standing guard at the door to the med bay, perfectly silent and still, models of military discipline. Build ideas because why not?

Here is a rundown of the builds I have on my two characters, simply because I felt like it. Maybe they'll give you some ideas with how you want to play. I welcome recommendations on changes. I'm also not that much of a stats guy outside of Crit so feel free to finetune to your heart's content. Without further ado:

Ongoing Directive:

I run Carnage and Pesti with Golan Chest with Trauma, Petrov Backpack with Wicked, and the rest OD. Skills are Mortar Turret (Demolitionist Spec) to proc Bleed more and Fixer Drone for heals. Three reds, three blues, a few Status Effect rolls and the rest red rolls. Good for close to mid range fights.

Designated Marksman:

Classic M1A with Breadbasket and Nemesis. Sharpshooter 93R (there's definitely better options from my experience). Punch Drunk, Airaldi Backpack with Versatile, Chainkiller, Overlord Gloves, Airaldi Holste, Providence Kneepads. Kneepads should probably be Fox's Prayer but I like to be different, and it's got great rolls already. All reds with HS Damage on all pieces with a few blues/yellows sprinkled (haven't found replacements yet). Fixer Drone and Revive Hive. Obviously using Sharpshooter Spec. Great for mid to long range and it's not that hard to take advantage of Versatile and Chainkiller if you position yourself well.

Medic:

Designated Hitter and SIG 716 with Reformation. Alps Chest and Backpack, Backpack has Calculated, Overlord Chest with Empathic Resolve, BTSU Gloves, R&K Holster, Hana-U Kneepads. All have Repair Skills and Skill Haste. One red roll with Technician Spec still gives you 6 Skill Tiers. Repair Hive and Chem Launcher for healing. Make sure with BTSU Gloves that you're canceling the Hive when it gets low to proc Transference Overclock. Stick close to teammates to give them heals and hit those headshots to proc Reformation. I use the Backup Boomstick with Optimist, works in a pinch. Positioning is all about being close to teammates.

Shredder:

Standard Adrenaline Rush/Intimidate build, with my own tweaks. I use Ninjabike pads as well because I play aggressive with this build and Ninjabike gives me boosts for doing so. Fenris Mask, Matador Backpack, Providence Chest, Ceska Gloves, Providence Holster, Ninjabike pads. Gunner spec with a Close and Personal Police M4 and Killer Infantry MG5 works amazing. I highly recommend Close and Personal because you get a range finder, helping you know if you need to close the distance. All Crit rolls with 49.7% Crit Chance and 59.

I'm the captain of an oil rig in the North Sea. We found something.

The divers boiled out of the diving bell, a wet mess of inky neoprene and rubber tubing. They looked like a mass of black snakes.

Oleg tore off his regulator and ripped the helmet from his head. It clanged on the steel of the diving platform, shattered faceplate glittering in the sodium light.

“Jesus fuck, Yevgeni – it got Alexei! It tore him…” He paused and threw up a mass of foam and blood.

I knelt down beside him, cradled his head.

“You didn’t decompress properly, Olya.” I could see bubbles of nitrogen moving through his skin. His scalp felt like bubble wrap as I stroked it. I knew I didn’t have long. He didn’t have long.

I turned his swollen face to me, locked his eyes with mine.

“What did you see, Oleg? Is the rig in danger?”

He wiped a trail of mucus and blood from his mouth and spat pink, foamy gobs onto the platform.

“It came out of nowhere, Zhenya – nowhere. And it was huge, captain! Massive! It – it…” he doubled over and clutched his midsection. The blue water in the bottom of the diving bell bay boiled and bubbled, streaks of pink shot through it as the divers coughed and vomited their ruined lungs into the swirling water.

They all died, one after another, within minutes, their bodies bent and broken by the pressure and nitrogen bubbling up in their blood. The whole dive team of 5 – minus Alexei, still out there in the deep - 600 meters below the wine-dark sea.

We met in the mess of the Aberdeen after the divers died.

I brought up the topographical map of the rig site on the monitor. It glowed green in the darkened mess. The 6 of us remaining on the rig gathered around it like Macbeth’s witches around the cauldron.

“We lost contact with the deep-water team here -” I said, tapping the screen at a point labeled Dunsinane Abyssal.

“About 80 meters from the Aberdeen. The team made it back – all except Alexei – through this channel, here.”

626 could image no worse fate.

He closed the tiny access hatch on the back of the Mechanek and jumped onto the wall, reoriented himself, and jumped onto the back of the next Mechanek, his vestigial wings fluttering to give him a little aerodynamic boost. The one he had just finished working on, Gunnery Sergeant Hunterson, jerked slightly as his brain finished synching up to his war-gear loaded body. Hunterson flashed 626 an icon of appreciation even as the little green mantid opened up the small panel. His next job, Sergeant Stoner, was waiting for his wargear to be checked out. He was having problems with his left side being tingly and his weapons losing synch.

626 ran the standard diagnostics, saw nothing wrong, ran a deep diagnostic, saw nothing. Then, remembering his lessons in percussive maintenance ran a physical check on the most problematic piece of hardware.

The braincase was 0.21mm from full seating on the left hand side.

626 held on with his grasping hands, jumped up, activated the graviton focusers in his little boots, and slammed down onto the braincase with 22.32 kg of force.

"Hey!" the Mechanek yelled. "Oh, nevermind. Thanks, 626, getting green lights across the board."

Humming the ancient war cant of The Triumph of Steel 626 slapped the hatch shut, locked it, and jumped on the wall then to the next customer. A Lance Corporal who's primary cannon array wasn't deploying correctly.

Still humming to himself, 626 kept working. Landfall was in an estimated five hours and he had many many human warborgs to run diagnostics on.

Why do my RCTs never work!? SO FRUSTRATED.

I've had 2 RCTs on 2 different teeth (47 in 2018 and just 7 weeks ago on 46). Both were irreversible pulpitis and not infections. 47 had to be extracted because it still hurt 3 months after I had it done, and I couldn't take it anymore. Now, the same thing is happening to 46. Been 7 weeks and I only had 3 weeks of semi-pain free living after 3.5 years of pain.

47 was done by a dentist I was working for, but 46 was done by an endodontist. I called the office and she told me that it's absolutely not the tooth. I am convinced otherwise. I am supposed to go for a crown prep on 46 this Tuesday and I am scared that I am going to spend all this money on something that is going to be extracted. I had 4 canals in 46 instead of 2 and she said there were no cracks.

The pain is radiating down into my neck. It's a deep constant 4-5/10 ache. Hurts when I floss, and is sensitive to percussion. I do have TMD and was diagnosed with myofascial pain syndrome this past January. I go for regular massages, physio, chiropractor, acupuncture and have medications that I take daily for these issues. I also see an oral and maxillofacial surgeon.

Why the fuck do RCTs never work on me? I just feel so defeated and want to cry. Is it so hard to just not be in pain??? Please help meeeeee.

Drizzlewood Coast - Special Missions Guide

Below is a detailed description for each of the 40 special missions unlocked via Special Mission documents earned by playing on the new map. Feel free to use this as a guide and not an absolute since these are the things I found to be the "easiest" to do. If you have any questions, feel free to mail or whisper me in game (Lama.2735). Good luck hunting folks.

Special Missions

  • Special Mission: Clear the ___________________
    • Kill 100 Dominion Charr in each of the six bases. This is a pretty easy mission to complete and I would let this mission be done naturally on its own.
  • Special Mission: Pillage the ___________________
    • Destroy 10 structures in each of the six bases. This is a very easy mission to complete if you use your Electromagical Pulse (EMP) on any structures including gates, barricades, walls, tents, and artillery. You should always be using EMP to destroy buildings to speed up the meta considerably.
  • Special Mission: Bolster the ___________________
    • Spend 600 War Supplies in each of the six bases. Use any War Supplies earned via events, chests, charr salvages, etc to upgrade or repair any structures. Easy mission to complete once you have lots of War Supplies to go around.
  • Special Mission: Cull the Herd
    • Kill 100 animals. Play the map and earn this naturally over time. I would not waste time going around finding animals on purpose. But if you want to be a rebel, killing wolves, bears, and minotaurs around the center of the map at Breakroot Basin is a good bet.
  • Special Mission: Cache Collector
    • Open 100 Cache chests. Completed easily by doing the Dominion Keeper "champ" train after the "Battle Cry" meta is completed. Each train consists of 40 chests but must be open with Cache Keys. Cache Keys can be purchased from the Quaestor NPC in any blue camps, earned via events, or gained every 10 minutes from base containers.

Ash Legion Special Missions

  • Ash Special Mission: Lies and Statistics
    • Destroy 50 propaganda balloons. This will probably be one of the last missions you will complete

Would I be a dick if I cut my friends out of my life?

(I tried posting this on r/AITA, but it said I should post here.) Obligatory on mobile...

So... I've thought about this a lot, frequently even, for months now about weather or not it's worth being in my current friend group. I mean we get along well and we share a few laughs but for some reason I can't help but feel like we don't actually like each other. Or at least that they don't like me. Now this isn't the reason I want to cut them out of my life though, that would be very petty, there's more to it than that. Sence middle school my friends have always been there for me, or at least they were there to listen to me talk about my problems even though I wasn't very "stable" persay in middle school. You see... my percussion teacher was emotionally abusive. She was constantly gaslighting me, calling me slow or stupid (I have ADHD so I'm in the special needs program), telling me I didn't belong at my school, tell me I'll never be a halfway decent musician and blaming me for the reason I had such terrible grades for three years (I could never prove any of this because she was a master with words and knew how to toy with them so it would sound like I'm reaching or making it up). Throughout the majority of these three years, I was very aggressive, depressed and constantly paranoid about going to my percussion class. And I would sometimes take it out on my friends. I was honestly the closest thing to a bully I've ever been. Now my friends weren't saints either but I was the most notably more unstable. That being said after an event in which I hurt one of my friends, my parents decided to get me professional help by the time I was in the second semester of 8th grade. And, it worked! I was calmer happy even sometimes, I wasn't as aggressive and was taking medication and going to therapy so by al intents and purposes I was getting better. And by the time I reached 9th grade I finally switched instruments and got a different teacher (he's great BTW) and now I'm no longer aggressive, just dangerously sarcastic. And I would do my best to make up for those two and a half years I would torment them by being the best person I could be.

Now on to the present. For context I'm a pansexual M. 19 year old cat with stage 4 CKD

Hey everyone,

Not sure how to start - I just joined today because I need someone to talk to/get advice.

I have a 19 year old tabby that I've had with me since I was a kid and I brought her over from Canada to Korea when I moved here after university for work. I've noticed in the past few months that she seemed to have lost a lot of weight and overall looking quite lethargic so I took her to the vet a few weeks ago.

The vet told me she has stage 4 CKD (high BUN/Cr, low potassium/sodium, very high phosphorus, normal calcium). Apparently its really rare to see a very old cat in Korea, so the vet has been very unhelpful. I contacted my cat's vet in Canada and explained the situation and he told me to get her on fluids immediately as well as famotidine and mirtazapine. My cat is also on azodyl and something to lower her phosphorus, but nothing more than that because apparently anything stronger will be too hard for her to handle right now.

In the past week after starting treatment, she seems a lot better - a bit more energetic, started grooming herself again and responds to be when I call her name or pet her. However, the problem is as follows:

  1. Even on 1/8 mirtazapine daily, she doesn't want to eat at all. She only eats little chunks of beef that I cook for her, but I don't think it's a viable option long term because it's not geared towards cats with renal disease. I've tried every option in the book and she doesn't eat anything...any advice on what I should try next? I've tried force feeding a liquid diet and she went into respiratory distress, so I really want to avoid that if at all possible...
  2. Her left hindlimb seems a bit lame - at first impression I thought her Achilles tendon had snapped, but upon physical examination, I don't see any swelling and she doesn't seem to be in a lot of pain so I'm not 100% sure now. I read online that electrolyte imbalances combined with peripheral neuropathy due to renal disease can be the cause of this? I would take her to the doctor to get it checked out but if it isn't a life threatening emergency, I am wary of taking her to the vet because she gets insanely stressed and I think it does more harm than good.
  3. There seems to be a bit of peripheral edema going on especially in her forelimbs. Upon auscultation and percussion, I didn't notice any abdominal distention or shifting dullness or any abnormal bowel sounds so I don't think there's any ascites yet. Should I just wait and see if the swelling seems

Tinnitus in one ear.

Hi everyone,

So I'm new to this community, mostly just looking for support from people who I know will understand.

When I was really little, around the age of 1-3, I got a lot of ear infections and have always been pretty susceptible to them since. I'm not sure exactly why or the technical terms for it but I had to get small tubes put in my ears because of that. I remember a couple trips to the children's hospital when I was around the age or maybe 4 or 5 to get my ears flushed out. My ears don't really produce much ear wax if any at all anymore, I think it's because of that. Growing up I never really noticed a lot pertaining to hearing issues. I knew from a young age that I had ADHD but my mom would never take me to see a doctor about it, she didn't really trust the medication. Anyway, I thought my issues with paying attention were the source of me not picking up on things being said either by a teacher, adult, or a friend. Never did I think "oh maybe I have slightly dampened hearing." Around 5th grade I got into music and band. I loved the percussion section, go figure. This advanced through three years in middle and then four years of marching band; drumline, pit & trap percussion, and concert band. Not too far into my first year in highschool did I start to wear hearing protection for all my musical endeavors. Not sure if that actually helped at that point because I'm pretty sure I didn't wear protection in concert band or for my marching band shows. I didn't notice a lot of ringing or anything in my ears in my four years of college but definitely noticed I had some trouble with my right ear in particular. I was the person in my friend group that would always have to said "what?", "Pardon me?" while leaning an ear forward, or the ever so frequent, "can you repeat that?"

Now I have a full-time job in cyber security. This job is great, I'm with fellow supportive co-workers, the company is small and I have an understanding boss. But this job is not without it's stresses like anyone else's. I worked out quite a lot in college, fitness was a way to cope with some other things I had going on in my personal life. Towards the end of college and the start of my full-time career I fell way off from my old workout routine. Just as I was starting to get back into the groove of things the pandemic hit. I had to work from home. I developed bad habits for sleep, not working out, and definitely posture.

Nursing 101: Caring for your loved ones at home

Your goal as a home healthcare provider is to build and support the immune system, limit the progression of the disease, and effectively communicate with local healthcare providers to determine if additional care or hospitalization is needed.

Start a medical folder or chart for each person while everyone is still healthy if possible. Have this on hand during phone calls with medical providers and for medical care/hospitalizations.

  • Birthday
  • Height and Weight
  • Medical history: diagnoses, surgeries, hospitalizations
  • Medications-prescription and herbal supplements: name, dose, frequency. For as needed medications like Tylenol or Tums, note the date and time taken.
  • Normal vital signs: Pulse beats per minute. Temperature. Respirations per minute. Blood Oxygen Level % (pulse oximeters can be purchased online and in drug stores inexpensively). Pain level on a scale of 0-10 including: location, duration, description. Tracking for seven to ten days at different times should be sufficient to establish a baseline. *Blood pressure readings at home can be inaccurate and cause alarm so unless you have a loved one with bp issues, I might skip bp. To double check an at home bp reading, you can visit your local pharmacy. You may also want to purchase a stethoscope and take an online crash course on listening to the various breath sounds.
  • Include: a chart with normal vital signs by age group, charts for proper Tylenol and Ibuprofen doses by age/weight. These can be given at the same time. Tylenol every 4 hours, Ibuprofen every 6. Some doctors recommend alternating every 3 hours to have continuous coverage.
  • Copy of ID and insurance card if you have one.
  • Research your medical system/insurance. Find out who your primary care doctor is, if your insurance covers telephone nurses or doctors, in home visits, urgent care centers, and emergency rooms. Keep these names, numbers, and locations in each folder.

When illness begins:

  • Note onset date, symptoms, and vital signs every 4-8 hours depending on severity.
  • Note calls to physicians and their advice.
  • Note interventions. As symptoms become more severe also document vital signs before and after to see if what you are doing is working.

Caring for someone with viral respiratory symptoms:

  • Wash your hands with soap and water before and after each contact. Wear a protective mask if available.
  • Keep the windows of the room open if temperature permits or air out room several times a day.

How do you feel about percussion massagers?

I’m looking to get one to help with scar tissue from a partial pec tear that hasn’t fully healed.

When I was in pt the deep tissue massage is what seems to help the most. I was going to start back up again with PT but with Corona, I’m going to lay off of that for a while.

I guess my question is do you first recommend percussion massagers at all( I’m pretty sure physical therapist use them but mine was on the cheaper side with little equipment so I’m not sure if that’s why he didn’t have one)?

And also are there any particular brands that I should look at? I wouldn’t love to spend the on money something like a Theragun but if there is a noticeable difference in effectiveness( I don’t care about loudness or look) then I have no problem paying the money to make sure I have a good product.

Interesting evaluation I did this afternoon.

Very interesting case I evaluated this afternoon.

44yo male presents with abdominal and LBP L>R. States that this past Wednesday he was helping his company deliver and doing a lot of lifting he doesn’t do because he usually works in the warehouse. His pain however began last night, not immediately after the injury or even 24 hours later. It’s primarily aggravated by moving around in bed at night. He also states that for the past 2-3 weeks his back has been hurting when he sits but he doesn’t know if they are related. Denies LE symptoms, numbness or tingling. No history of LBP.

States that for the last 2 days he really hasn’t been moving, eating, drinking or doing much of anything other than work or sleep. He thought he was spiking a temp but his wife checked it last night and it was normal. He also noticed that he has been urinating a lot despite not drinking that much water and that his urine is dark, no trouble initiating a stream and no pain urinating. No bowel issues.

Objective:

Lumbar ROM WNL pain free in all directions

Via squat screen B LE ROM pain free WNL

B hip ROM in supine WNL pain free

No TTP across pelvis, Sacrum or lumbar spine

Strength B LE and hips WNL

TTP Left lower quadrant (LLQ)

Percussion to LLQ painful

Rebound tenderness LLQ painful

Hard mass felt in LLQ

Thoughts?

I referred him to the ER with concerns for kidney issues. Definitely non mechanical LBP.

The fucked up part? I work in a workers comp OP setting so he was evaluated by a physician yesterday (literally less than 24h ago) and subsequently referred to me for LBP. I feel like a lot of people in this field forget that we really are medical professionals.

Pneumonia advice, for a super stressed parent

So, my 16 month old daughter started coughing and producing a lot of mucus. We took her to the hospital to get an X-ray on her lungs. We live in a country where very few drs have ever really seen a CF patient.

So, we get the x-ray back and there are some spots in her lungs that indicate pneumonia, we do blood work (We just had blood work done 3 days before when the symptoms started, it was actually the day we found out she had Cf). The blood work indicates its a viral infection. Looking at the blood work from 3 days before it indicates it has pretty much run its course. You could hear the mucus in her left and right lung. They tell us to just nebulize her more and introduce us to percussion therapy.

We started nebulizing her every 4 hours, once in the morning with pulmicort and 3% saline, then 3% day and night and then pulmicort and 3% before bed, even waking her up every 4 hrs to nebulize. We also did 15 mins of percussion everytime.

We take her back to the dr 3 days later and her left lung had cleared up and the right one sounds better as well.

On the last post I made, about things that you wish you had known, someone said you need to give antibiotics every time they get sick.

Should that be the course of action? Should we give her antibiotics?

The dr said we dont want to give her antibiotics for small things like this because it starts a vicious cycle and sets back her development. (This dr has seen patients with CF but not in over 20 yrs.) She had severe pneumonia a year ago and it really set her back, she did 2 rounds of antibiotics.

What would you do in this situation? Ever since my last post I have been stressing hard core. She seems to be doing better but is something colonizing? Do these dangerous CF bacterias show the same levels of white and red blood cells as viruses? I feel more and more lost with this disease everyday. I don't feel like I can get the proper medical care here but can't go to the states because my wife isnt a US citizen and everything has been suspended here. I realize this isn't medical advice, I just want to know what poeple who have dealt with this would do, so I can talk about it with our dr. THANK YOU

A Story w/ a Happy Ending (severe covid)

I’ve been resistant to posting our story on this page because I wasn’t sure how it would pan out. In many ways, I still don’t know how it will pan out, but I do know that 1) we are a far cry from where we were a week ago, and 2) as the despair starts to settle deep into our collective psyche, it might help you to read a story like ours. A story in which the protagonist knocked on death’s door, and maybe even opened it a little bit (he has always been the curious type), but walked away in the end. “Not today,” he said to the god of death.

My partner Laufey (name changed for a bit of privacy, account is also a throwaway) and I live in the current epicenter of the global pandemic, New York City. Laufey is a public school teacher in the Bronx, and I am a research scientist at a Manhattan university. We are both in our early thirties with no preexisting conditions. When the pandemic started to hit our city, many of our friends started to file out of the city to wait it out in more rural parts of the country. Though we technically had the option of leaving the city and going to my parents’ house down south, we decided against it for a few reasons. One, my parents are both high-risk, being in their seventies, and we didn’t want to bring the virus home to them. Two, my parents don’t totally approve of our relationship. We are an interracial couple who are premaritally cohabitating, and they are extremely “traditional.” We just didn’t want to expose ourselves to weeks or months of their judgment. And three, we figured that if we did happen to get infected, we’re young and healthy enough to fight it without too much of a hassle.

Boy, were we wrong on that last bit.

Our symptoms started on Thursday, March 19th, but we didn’t recognize them as covid symptoms until days later. I woke up that morning with total anosmia. I flagged it as weird, but I just attributed it to allergies, since mine can be severe. I took an allergy pill and went about my day, which involved going to work one last time to wrap up my experiments. (Spoiler alert: The allergy pill didn’t make my anosmia improve at all.) Laufey woke up with a headache and felt panicked, but he thought it was all just stress. It was the end of a very tough week for teachers, and he was amidst the challenging transition from teaching seventh graders in person to teaching them online, during his first year of being a teacher no less. What Do You Love About Your Town?

Hi! My in-laws are planning to move to Winston-Salem from Southern CA in the next few months and are practically begging us to follow them. Even though both of us were born and raised here, grew up in the same suburb where we are now raising our pre-teen daughter, and love a lot about SoCal we're ready for something different. If you want to have a comfortable life in CA these days you have to sell your soul to a corporation or be a trust fund baby, and we're not trust fund babies! We want a slower pace of life, a nice yard for our dogs to run around in, and seasons!

So I've been looking at subreddits like this and for the other Triad cities, and you all make it sound like such a lovely place to live, we're really interested about learning more! I'd like to know more about what it is you all love about your area, and what makes it a great place to live. I'd especially like some comments on what the job market is like out there (other than what I'm sure has been a horrible crash due to COVID). Out here it's hard to get anything other than an entry level position unless you personally know someone who is doing the hiring...is it like that there for office jobs?

A little about us: I'm a Project Coordinator for a Property Management construction team and he does administration for a Johnson + Johnson medical device sales subsidiary... so we're both seasoned desk jockies. Our daughter plays percussion in the school band, and sticking with band is important to her. Are the school bands pretty competitive out there? Okay, before we begin in the shop, since the rest of you have been familiarizing yourself with Newton's Laws, the laws of physics, and relativity, I'm going to add some for the engineers here: Moriearty's Laws.

Anyway, we'll begin as they were originally written.

Law One: All tools are hammers.
This may lend a lot of insight into human psychology. No, not many tools are intended to also be a hammer. But they all are. Believe me. It's a human thing.

Law Two: A tool's use as a hammer can be measured inversely against its actual usefulness in its original, intended, specialized purpose.
Example: A big wrench is also a passable hammer. Digital calipers? Not much else does their job well, and they're terrible hammers.

Law Three: A tool's use as a hammer is likewise inversely measured against its cost of replacement.
That wrench? You're not going to break it, and you can buy another within walking distance on any colony or station without going into debt. A good rechargeable drill makes a decent hammer, but you're going to owe your maintenance department.

Law Four: If Law Three causes debate, usefulness as a hammer includes the cost of medical treatment.

When The Gods Come to Visit - Chapter 23

It was early evening as Glenn ran through the streets of Ploria with Kal’Eva on his back, following Issa’Vala who was a few steps in front of him. Quietly to himself, he lamented that he didn’t take an earlier interest in the ‘healers’ of Ploria. He was too concerned with getting the ovathi away from subsistence farming and advancing their tech that he neglected improving their health. Although he supposed they were ahead of the curve when it came to hygiene at least when compared with humans, they seemed to bathe regularly and with actual soap, not oils.

Regardless, he had no idea what symptoms a sick ovathi would have, or how their immune systems worked or anything like that. He, along with the rest of the crew back on the ship, were specially treated in a rather uncomfortable medical procedure that eliminated anything within their bodies that could potentially infect an ovathi. Although the chances were really quite low that anything could, it was better to err on the side of caution.

As the two moved through the city, Glenn noticed that they were not heading towards the temple. He thought he remembered a goddess of some sort of healing, so why weren’t they…? Oh right, the goddesses do not interfere in the affairs of the mortal realm, if you exclude him and his crewmates. The goddesses were seen not as supreme, powerful beings to fearfully worship but more of an example to follow.

“It is just around this corner!” Issa’Vala called out to Glenn, snapping him from his thoughts.

Glenn didn’t know what to expect from a healer’s place but it wasn’t this. It was a small building, only a single story, painted in bright red and brown hues with a very obvious symbol above the doorway. It was the sign of the goddess Glosi, if Glenn wasn’t mistaken. A kind and knowledgeable goddess.

“Erm...has...Kal’Eva ever been here before?” Glenn asked.

“A few times, I think,” Issa’Vala answered as she started to knock on the door. “But trust me, Ay’Wa is one of the best healers in Ploria. She has been the healer for my family for a very long time. She is very knowledgeable.”

Glenn wasn’t so sure. Wouldn’t it have been better if they went to a healer that Kal’Eva went to regularly? But then again

“Excellent. Shall we begin?” Dr. Offerson picked up his clipboard. Little holograms and readouts flashed above it.

“I’d actually like to start a little before. The Temporal Defense Initiative loves their, uh, oldschool interrogations.” Jay tried to think back through it all.

“I want to see my child,” Jay groaned as two temporal defense agents dragged him to an interrogation room and roughly sat him down at a table. “I want to see my son.”

One of the agents cuffed him too tightly, of course and stood back as somebody walked in, sitting down on the other side of the table. It was an alien vaguely resembling a demented, six-eyed kangaroo.

“Jason Tersk,” the alien- Jay didn’t know what kind- said slowly. “You are accused of endangering the temporal security of the Council of Eight Hundred. Do not bother pleading, we already know you are guilty.”

Endangering the- Jay knew for a fact they weren’t gonna tell him what that meant, so he didn’t even try. “I want to see my child. My wife just gave birth to him. Please. I want to see him.”

Alright. I need some serious fucking help guys. What the hell is ‘Siren Head’, and how do I stop it?

I guess it’s only fair that I give some sort of backstory. Every year, my friends and I go on a camping trip. We’ve always been competitive, so one year we decided to turn our camping trip into a competition. One of my friends used to watch some survival show where two individuals are tasked with surviving a month in the wilderness, and in our infinite wisdom decided to replicate this with our summer camping trip. Over the years our routine had become pretty cyclical. I’m the group fuck up and transient so naturally I had to buy the required hippy wagon to match the mantle. This makes me the best chauffeur for these trips. I drive around and gather up everyone and then drive us to our spot where we start the competition. From here we all decide upon the rules. For instance, one year we decided to make the special rule that the last one who makes it back has to buy everyone’s weed the next time. The only rules that stay consistent are as follows: 1, the first one back leaves the van. We keep a dirt bike on the back of the van if you need to go into town, but leave the van. If gods forbid, we have some kind of medical emergency we would want the van to take the individual in question back in town. 2, you can only bring one piece of survival gear. It can be anything, but you can only bring a single item of it. We also keep radios on our persons at all times, but each radio can only be used to contact the ‘command center’ I had rigged up in my van. This way if we decide to take someone to the hospital we can still keep in touch with everyone still way out in the woods. This year everything started out copacetic, but things got really weird about three days into the trip.

This year, I was paired with a buddy from my old volunteer fire department days. He’s not the brightest crayon in the box, but he’s funny and he’s got a good heart. He decided to take his old Halligan tool he had used while we were still part of the department. His name is Joe Arceneaux. He was a tough-talking bayou boy who supposedly had gang affiliations back in New Orleans. I called myself cheeky and brought the keys to my van. We all drank out oaths to good fun and trekked to our designated starting points. Our first day was pretty routine. First, reconnoiter the surrounding area. I’ve really wanted a pen pal for some time and finally making a post about it. I have a lot of interests and hobbies so bear with me. I tried being thorough-

For school extracurricular: cross country, distance in track, philosophy club, and earth group. I’m decently studious and take all ap and preap classes available. I try keeping up with current events and listen to A LOT of podcasts and at this point watch 80% of decent Netflix content in the slice of life, romance, drama, horror, and thriller genre movies/tv shoes. Always open for more suggestions. I do watch anime but not that much (art slaps

For hobbies I paint/draw, play percussion (mallets and snare but I have a drum set I haven’t used in awhile), garden, run, bake, cook, take photos, dress in lolita (it’s a Japanese street fashion), read (I have a small book club with two classmates), and do some crafts (candle making, cards, etc). Oh, and I play a little overwatch but I’m pretty average don’t do competitive (I just do it to support one of my friends). There’s more I probably forgot-

My interests are philosophy, psychology, self improvement (trying to loose weight, eat better, working on better habits etc), politics, the outdoors, environmentalism, anything plant related and general gain of knowledge. I also want to become fluent and Spanish so I take it at school.

It’s kinda hard to describe my personality but I’ll try: I’m very passionate, determined, I can get a bit cluttered in the head and with my stuff when stressed, and I’m the mom friend. Back when we were still at school I would bake in bulk for my classmates and friends (like cookies, cakes, cream puffs, etc). I’m also kind of a cliche cottage core girl with my obsession of nature and plants. Sometimes I’m intense other times I’m light hearted and just living to live. I’m very mature for my age (feels weird to say but if you get to know me it’ll become evident). My job I’m striving for right now is to become a lawyer (medical and then maybe criminal end of my career so I can help others pro bono). I’ll probably end up near a city but when I’m experienced move out a bit and get a cozy cottage.

I’m a busy person and if possible I’d love to do two letters a month!

If all goes well, I will be the last human to remember being born.

I don’t remember all of it—the quiet, safe slumber inside the womb, the towels and fussing, the injections and implants—those just blurred together, although I’m sure the process was extensively documented by the military scientists present at the birth of my siblings and I. But despite what was lost to the mists of time, I still remember the highlights.

I am only human, after all.

When I was born, I cried exactly once. Less of a cry, really, and more of a mewl. It was hard to keep crying, though, when I was surrounded with so many wonders.

Lines of babies extended to either side of me, for as far as the newborn eye could see. I think my senses were still a bit muddled up—they looked like the smell of a hospital, and their cries tasted like lemonade.

A sound caught my attention—the child to my left was burbling. I listened for a few heartbeats, then gurgled back:

“Glblglb?”

The other child stared at me, surprised, then smiled, hands waving in the air. “Dwee-bull! Ybblptth sllmpbbl.”

I nodded sagely. Wise words, for one so young—for they were words, indeed. Words I hadn’t deciphered yet, but words which held a meaning nonetheless. I tapped myself on the cheek and said, “Eurybmph bloblahcha. Ybbltth blo—”

Please help me type myself. I'm lost

I would like to think I'm just a normal human being with no quirks. I'm currently taking a software engineering course at a local university. I guess I'm generally a quiet and reserved person. I have the appearance of a reserved, well-mannered girl. The two most common first impressions of me are: 1) good kid. 2) very serious and a bit intimidating.

2) Is there a medical diagnosis that may impact your mental stability somehow?

no. I don't see myself as someone who has serious mental issues. I do sometimes have dark thoughts but that's normal for everyone.

3) Describe your upbringing. Did it have any kind of religious or structured influence? How did you respond to it?

my parents are the religious type. and I'm also someone who tries to follow my religion religiously (keyword: tries to). I went to religious schools: kindergarten, elementary, and high school. so I'm pretty influenced by my religion, morally and ethically. if that makes sense.
I would say that the teachings of my religion sometimes plays a big role in my decision making. and they are also my moral guidance I guess? a girl just wants to get into heaven yknow.

4) If you had to spend an entire weekend by yourself, how would you feel? Would you feel lonely or refreshed?

definitely more refreshed than lonely. im really fine spending time by myself. having plans are actually dreading to me. although, sometimes, having fun with my close friends is also something that I look forward to. but if the plans were to be canceled, i would not be devastated :)

5) What kinds of activities do you prefer? Do you like, and are you good at sports? Do you enjoy any other outdoor or indoor activities?

Q: So far the reports have said that only elderly people with a weak immune system was in the highest danger of dying. Is this changed by the fact that a 44 year old has died in the Philippines?

Q: Is it a good idea to wear a mask that covers your nose and mouth when you are at the airport or in the airplane, or is too drastic?

A: It’s too drastic.

Q: Why should we be worried about coronavirus when a regular influenza killed 1500 Danes in 17/18?

A: We shouldn’t be worried. We do take new viruses seriously and try and learn from it. The country around us all take different percussion measures and we try to contribute to some reasonable precautions.

Q: How high is the risk to be infected with corona in Denmark?

A: The chance of being infected in Denmark at this time is low. All indications are that the disease is not quite as serious as first feared. Denmark has a well-functioning healthcare system. So there is no need to be afraid. (My comment; Hospitals across the country have a shortage of nurses so much so that regular surgeries are being canceled. The waiting lists for a surgery can be long, and nurses in the big cities are often overworked)

[Discussion] Should I disclose my disability?

I am a recent community college transfer with single sided deafness. As you can imagine, it's a little difficult keeping up with lecture when you can barely hear the professor. In small classses of 30 at community, this wasn't a problem, but I struggled immensely adapting to the large lectures at my university and failed 2 classes outright.

Since then, I've gotten accomodations from student disability and have retaken those classes with As. However, I'm worried that this looks terrible on my application - a community transfer who can't hack it at a 4 year university. But at the same time, I don't want treat my disability as an excuse and turn my app into a sob story. Any advice on how to approach this? Is being deaf going to be a huge problem as a doctor?

I can’t say much really ties them together (even in terms of electronic Vs. acoustic) other than being beyond popular music by a good measure.

No expectation these will light up the dormant commentary, but I hope they’ll intrigue and challenge you as enjoyably as they did me. Why not try something totally different?

  1. “Outliers” - Sybarite5 (2019). Not to be confused w/ the also excellent, electronic Sybarite on Temporary Residence, this is a highly enjoyable album offering a full hour for reading or close listening. Not entirely new classical, as there’s a stringy blues number, but that’s the overall flavor even among what may not be their usual output (if we take the title at face value). Very nicely done. Apparently they’re best known for rearranging Radiohead for chamber quintet. The album is in turn subtle, sweet, and forceful.   3. “Plafond 6” - Cucina Povera, Haron (2020). The rest of the Plafond series on the BAKK label wasn’t as enticing as these soothing, low female vocals over electronic sound beds, though they’re all very likely worth a 99-cent download based on length alone. I can’t tell if they’re singing lyrics in any particular language and would rather not scrutinize. The third track on this one, Riffitelyä III, is the one to judge the album by. IMO, it’s what takes the whole piece from moderately pleasant and interesting into the sublime range. I’d liken it to the softer work by Loop Guru, Tom Middleton/Global Communication, or even a vastly slowed-down When Saints Go Machine, though I’m sure there’s a more apt comparison out there. Entitled Ex-Friend Tries To Get Me Suspended...Twice

Alrighty, so I'm sorry if this is long. This also isnt fulll out entitlement, but im not sure what else to call this.

So im gonna call this entitled ex-friend Zoe for the purpose of this story. Let's begin.

Just after 5th grade, i moved to a new school. I left because of bullying (physical and verbal) from pretty much everyone at school.

In 6th grade i started at my new school, and it was tough having to learn the layout. I gained a few friends rather quickly, one being Zoe. Zoe was more of an outcast, and even up til graduation, people only barely tolerated her. I found out why fairly fast.

Luckily at this new school, i wasnt really outcasted like i was at my old school. Things were plenty better. I even got along with the teachers better and they actually cared when someone was bullying/etc. (I had been called to the office many times as i was witness to fights and bullying constantly, but it was good because they actually took care of it)

Sometime around Valentine's Day, i think it was 7th grade, Zoe's parents wanted to go out on a date. Her mom kept pushing and pushing that my mom should babysit Zoe. Now again, were in 7th grade. We're at the age we're plenty old enough to stay home alone. By this point, i had spent a night or two by myself at home. And Zoe had siblings, all older, so it's not like no one would be home or on standby. Zoe's mom was an EM, and it made the rest of this pretty obvious as to why Zoe was entitled.

Come to 8th grade year, me and Zoe were still friends, tho pretty rocky. We had plenty of classes together. This year, i also made a new friend. We'll call him Trevor. Trevor didnt have a good home life, at all, and somehow we got close enough that he felt comfortable talking to me about it. He'd vent and rant and i'd just be there to support him. We had gym(first half of the year) and art (second half) together, along with Zoe. In gym, me and him would make sure we were on the same team just so we could talk if he needed to, during down times in games. Also during gym, Zoe, who i know has diagnosed mental illnesses, would act out pretty heavily. Enough so that teachers (even in classes outside of gym) would send her to the office. Now, i had been to her house a few times. She took medication, but its like it didnt work. And me, having high anxiety, would always get really anxious when she would do these things. She'd run around the room (again, even outside of gym) screaming "Im a bird" and flapping her arms. This obvi

The cluster of amethyst in Eol's right hand looked like it had just been picked up from the shattered remains of a large geode. Its base was a finger-thick shell of textured grey-brown basalt. Most of the shell was occupied by three large euhedral crystals. Each was slightly different in height, circumference, and the angle at which they protruded. The interior of the shell in between and around them was crusted with a thick druse.

The basalt was mostly flat. What gentle curves it had fit perfectly into the central crease in Eol's palm, between her two carpal bones. The shell had, apparently by chance, cracked in such a way that it left two rounded dimples on either side of the cluster. As it happened, her inner and outer thumbs hooked smoothly into those dimples. With her left hand, Eol distractedly traced slow, complex patterns on the three major crystals—one finger per crystal. Occasionally, she would tap the druse with her left inner thumb.

Eol tilted her head and laughed at whatever Moull had just said. Moull was carrying around a tiny potted tree. He held it in one hand, and brushed the tree's long blue needles with the fingers of his other. Occasionally, he tapped the rim of the pot with the inner thumb of that hand.

Eol briefly considered lifting her amethyst cluster and bashing it against Moull's head. The [millimeter]-thick gravity strips ringing the base of the geode would easily compensate the motion. Moull building his datalink into a living plant was just—it was too much. Easier in some ways, she thought. The major crystals of Eol's cluster were large enough to be semitransparent. Anyone could see they hadn't been drilled into or modified in any way. It would have been childishly simple to manufacture fake crystals with circuitry grown invisibly into them. Many Sward-rank Qinis did just that. Or, they had done so three seasons ago, when the fad had swept through—hopelessly gauche, like all Sward flights of fancy. Today, many of those three-season-old castoffs were trending among the Loam ranks.

A Copse like Eol or Moull would sooner step off the nearby balcony, a full [three-quarters of a mile] to the pavement below, than use a datalink whose form was not absolutely, thoroughly genuine. Of sounds and scars

Hello, I have not forgotten about "When the Neighbors Swing By", but I only have so much time and attention for writing and this story has been jumping up and down demanding to be written. So off we go.
Edit: Warning, this story gets dark.


Throughout the galaxy one of the things that humans are known for is our remarkable healing speed. This has allowed humanity to do things that most other races would consider overly-complicated suicide; we just call it surgery. However there is no such thing as a free lunch. Humans pay in allergies, auto-immune diseases, and scars.

It was the near the end of a long exhausting day. As a member of the Terran Alliance trade delegation I had spent the day with my colleagues trying to hammer out an agreement with the Qawi Union at one of their "Resort Stations."

We had rather easily come to mutually beneficial terms on raw materials, manufactured goods, and my specialty agricultural products. The Qawi have a thing for spelt.

We were ready to move on to the intangible, and the Qawi insisted on starting with music. They took music in all its forms quite seriously; to be a musician in Qawi society was seen as a nearly spiritual calling. Songs were classified according to their color.

They said the color of music is quite self evident. To prove their point, they played samples of what were considered the ur-examples of each primary color and had each team member write down the color. Since their eyes evolved similarly to ours, the primary colors were Red, Green, and Blue.

To my admittedly under-trained ears they all sounded similar. The first sounded remarkably like an out of tune flock of geese enjoying a tour of a cider mill. The second, ducks astonished by a sugar beet processing plant. And the third trumpeter swans caught in a cotton gin. It was enough to bring on a migraine complete with nausea, but the pain was at the back of my head.

The guesses of the team were completely random. Only Laura, our music expert, got all three right. Even then, when she started playing examples from Terran music, her expectations did not match the Qawi's assertions.

I (21m) had to pull the life support on my (62m) dad.

This was a few weeks ago but I think about my dad a lot.

My dad suffered cardiac arrest. Between ambulances being slow, EMTs not having right equipment on them (yea that's a whole other issue), etc. there wasn't oxygen to the brain for a good 12+ minutes. He was taken to one hospital.

I got out of an observation (I'm an education major and have to observe real classes), and see I have a million misses calls. I call one of my brothers back and then yeet to the hospital (an hour away).

My mom, brothers, aunts, uncles, etc. were all there to visit my dad. They transport him to another hospital an hour away to get another opinion on his brain and heart, and to possibly have surgery to fix a split valve. Just one of my aunts and I drive up there. (We all opted for my mom to not go..she's got her own medical/mental things ..If she was there, we'd have two emergencies). Just one of dad's sister's and me.

At this second hospital, they confirm he's brain dead (and therefore not a candidate for surgery). It was ultimately my decision to pull life support on my dad. My mom isn't mentally or emotionally able to make decisions, as asshole-ish as that sounds.. My brothers legally aren't my dad's, since they were from previous relationships my mom was in. Rest assured, he sure as hell raised them like they were though.

I know it was the right decision. He was a vegetable and there was no use in continuing that. Being that school is online and sparse now (Music school doesn't work so well online haha), I just have a lot of time to think about my dad and I have a lot of time to experience him not being home.

I did play a song on piano at the end of his funeral service ("Wind Beneath My Wings"), which give did some closure I guess, since he'll be missing the rest of my performances - including my junior percussion recital, and my first cantata as a choir director (which are both cancelled anyways due to current events).

I don't know. It's just weird more than anything. People keep telling me how strong I am for making decisions, planning the funeral, picking the burial plot, making all the phone calls, etc. Not to be toxicly-masculine, but I don't really have a choice to be strong, y'know?

Trip report: Turkey, Bulgaria, Romania, Moldova, Ukraine, Belarus, Baltics, Russia

About me: I was the OP of the this post. I basically went from Istanbul to St Petersburg through all of the countries in between. I want to write this post for anybody else who is considering something similar or going to some of the countries I visited. On average, I spent less than a week in each of the countries. There were some places I stayed up to 10 days (e.g. Romania) and others where I only did one night (Estonia). So obviously I am not an expert by any means, and barely scraped the surface for any of these places. For context, I am a 22 year old, male Australian university student who spent six weeks travelling from Istanbul to St Petersburg by myself. I had read quite a few books about travelling Eastern Europe and the history of many of its countries so I was super interested in visiting. Before this, I had been to Europe twice, but only went as far east as Poland. After writing this, I realised that it sounds a lot more like a series of journal entries than a report. I really enjoyed writing this, and if anyone has any questions, please feel free to ask me. I will be more than happy to answer.

Budget: I didn't really have a budget while I was travelling. Of course I would try and find affordable places to stay while I was travelling and even couchsurfed a few times. Overall I spent about $2000 AUD (not including my flight to Istanbul) over six weeks. You could definitely do it cheaper. That amount also includes my flight in and out of Belarus. Food is quite cheap in that neck of the woods. If I wasn't couchsurfing, I was staying in hostels where I was spending about $15 AUD on average per night.

Transport: Getting between places is quite easy and affordable. I mainly took busses (Flix and Lux Express) and marshrutkas (crappy shuttle bus that is a relic of the Soviet times). I think I only took two or three trains while I was there. I booked about 70% of my transport at a ticket desk on the day before I was scheduled to leave. If you want to go by marshrutka, you probably won't be able to find tickets online, but it is super easy to buy tickets at the central bus stations. The cost of transport ranged from $1.5 AUD (marshrutka in Bulgaria) up to $20 AUD.

Well this is a thing were doing as were cooped up inside. For this I took what I thought to be the 30 biggest and most iconic hits and ranked them, using my 30 point scale of instrumentation, vocals and lyrics with an extra 10 point for overall impact on the industry, I’d love to know how you view it, but first a few clarifications, this is a ranking of the biggest hits, some you won’t like, even I have distaste for a few, I’ll save the best songs for later down the line, I’m just taking each hit and ranking them worst to best, I’m not calling these 30 the best just the biggest. Also one song per artist as a certain chief would probably overwhelm this top 30 and a certain duo that must not be named and has grown an overall distaste from people over the years would also swarm it with probably 10 songs. So lets get started shall we.

30 . Peter Pan-Kelsea Ballerini

Definitely one of the more obscure ones on this list but it was a huge hit in its own right, a lot of people have praised it, I don’t think that at all, a lot of lines where just copied and pasted to give the illusion of rhyming because overall the song is a mess, is this a reflective song, a power ballad, this song doesn’t decide what is half the time, vocally it is overall drab and bland. I don’t see the huge appeal of this song, its not that catchy and the overall package is the reason its dead last.

  1. Die A Happy Man-Thomas Rhett

Ah yes Thomas Rhett, every girls dream boat and every man’s dream to kill him and dump him off a boat. Thomas Rhett’s second single off his sophomore album became the biggest hit of his career, this song exploded, sure it was an Ed Sheeran rip off, and I’m an Ed Sheeran fan but it is a solid rip off, it is sweet though hollow, it is way to checklisty. If you locked me in a room and made me come up with a song based off this premise this would be the result. Lyrically it is pretty shallow and often was the production even though I heard a fair amount of steel throughout it. Die A Happy Man was seen as the biggest hit of the 2016 until H.O.L.Y came around swiped that away from him. Die A Happy Man accomplished what it went out to do, I still listen to it to this day, and I say a large amount of people still do.

(Dentistry) Tooth seems to be a routine extraction based on xray and special tests, yet proves to be impossible to move at all?

Hi, relatively young dentist here, only 3 years of experience. Now, I am certainly not the best with extractions - learnt a lot on mistakes and have done probably 30 not more after school. That said, didn't have a single problem for the last 2 years. I could do complicated (broken, heavily filled, heavily decayed, RCTd, etc) extractions as well, although slowly. Never had to do a surgical. So I felt I was competent enough to do routine extractions. But it all changed yesterday.

Elderly male, 60 - 70. No relevant medical history. Presented with pain, diagnosed with irreversible pulpits / partial necrosis with periapical infection of lower left second molar. Xray showed nothing unusual - average root length, parallel roots, PDL visible. Percussion sound was normal - no suggestion of ankylosis. So, after treatment options were discussion, we decided to go with extraction. Everything screamed to me a normal routine extraction of lower left second molar.

So I numbed the patient up - no problem. Tried to elevate the tooth. God... There was no 3rd molar so I could only elevate mesially. I had no problems elevating before. But this tooth. I could not even rotate a bigger instrument between the teeth at all! After nearly 1 hour of putting a hell a lot of force into elevation / luxation, I gave up on forceps after few tries - the tooth did not move a millimetre. How is that possible? Normally after elevating and pushing, eventually it should become mobile due to haemorrhage and crushed PDL. I would expect Grade 2 mobile at least after so much elevation. What can be the problem? I definitely used a lot of continuous force.

[Mod] Playing as humans

I've editted the raws to make humans playable, and be able to make steel, and have scholars.

I also created my own nobility system, without the whole King/Duke thing because I'm not a fan of monarchism.

Basically:

- King becomes Marshall

- Marshall is succeeded by the General, who is [Elected]

- Added [ATTACK_ENEMIES] and [PATROL_TERRITORY] responsibilities to the military responsibilities. This makes Worldgen a lot more interesting.

- Added a Shaman who has [RESPONSIBILITY: RELIGION] and [MEET_WORKERS].

- Chief medical dwarf is now Chief Healer

- Replaced Manager/Bookkeeper by the Union Leader, combining their responsibilities and adding a [MEET_WORKERS] responsibility

I tested it, it should run fine.

Tool's multi-album "movie" theory - new details

For the last 2 months, I have been sporadically posting about how Tool's tracks all the way back to Opiate can be mashed up and reconfigured to create a cohesive narrative. Around 3 weeks ago, I deleted my original reddit posts and soundcloud links. The original posts explained how Tool and their audio engineers have chopped up the frequencies that make up different sounds and placed them closer together or further apart while also placing some of the frequencies on tracks from other albums. When you mashup the tracks and change their speeds, some frequencies come together to create new sounds/lyrics, whole others move apart to essentially dissapear into silence and echo effects. Some frequencies of hidden words are so close to percussion or instrument sounds that they are completely hidden until manipulated with audio software. Put on some good headphones and listen to the beginning of Pneuma. Do you hear the words poking out above the initial guitar chords? "We've already recorded a thousand albums ..." is how it starts.

I finally was able to hear most of the giant movie plot around 30-seconds at a time on the new PC I bought for this purpose. However, nothing is lined up precisely enough for me to post. You have to quantize and warp tracks using audio software to match beats, and it is very time consuming. However, it is worth it because entirely new instruments, sounds, and lyrics appear. Some of the tracks are sound foley and dialogue, not even music at all.

I'm positive that I "solved" the "movie" correctly because once I figured out the ending, the entire plot snapped into focus and made sense. A Perfect Circle's albums and Puscifer's albums are important parts of the movie too, providing audio for the movie, fleshing out some of the characters, and providing clues on how to solve the storyline. The song "Goodbye and Thanks for the Fish" is one of the songs that plays during the end credits. That doesn't spoil anything. Even some of Maynard's vocal collaborations and Tool's opening acts in the 2000's point towards aspects of solving the movie. Parts of the ending of the movie occur in some of Tool's tracks early in their discography, which means that Tool has been planning this and keeping it a secret for a LONG time. The way they used the technology and songwriting is so complex that this is very likely the most complex musical work in human history. Album Background

I was 16 when I first heard about Mac Miller. Walking down the hallway in between classes at my old high school, I noticed a pile of flyers laying on the floor promoting a gig for someone who was, at that place and time, just a local rapper one year older than myself, trying to make it big in a city that was, up until Wiz’s ascent, not known for rappers or hip hop music of any sort. I couldn’t have predicted that the kid I saw on the ad, performing in a small venue that likely didn’t seat more than a couple hundred people, would have the first independently released Billboard #1 in nearly two decades by the end of the following year. I couldn’t have predicted that he would be making music with some of the biggest names in the industry not long after that. I couldn’t have predicted that I would meet Mr. Miller myself on a couple occasions and, especially considering his earlier material pre-Watching Movies with the Sound Off hadn’t wowed me, that his later music would someday resonate with me in a way that not too many other artists’ music would.

 

I also couldn’t have predicted that eight years later I would be legitimately shaken by his passing, more so than any other musician or public figure who had left this Earth earlier than expected in my lifetime.

 

The tumultuous final months of Mac’s life were well-documented, as his break-up with Ariana Grande and subsequent drunk driving incident came within a week of one another in mid-May. It was at this point that I, along with plenty of others, became genuinely concerned for where Mac’s headspace was at. The drunken joyride was only part of the problem, as Grande confirmed that Mac’s inability to remain sober was what ultimately led to their estrangement.

Teaching music a good extracurricular?

Hi everyone! I'm a second-year undergrad student, just switched into a pre-med major, so I'm a ways off from even making a school list or knowing what my stats are gonna be. The main question I have is whether I should continue with my music teaching job. I teach percussion year-round at a high school in the area for middle and high school students. I really enjoy my job, but it takes a lot of my time (15-20 hours a week) and I'm not sure if I should keep doing it or spend that time doing other things like research and/or clinical experiences. Right now I'm volunteering at a hospital (4 hours/week) and working in a chemistry research lab (10-12 hours a week). Is teaching an extracurricular that medical schools will look at favorably, or should I do other things instead?

Legion Rework Concept

Legion has been reworked and tweaked over and over and each time the "Legion" has been a single killer with a kit that isn't that fun to play or play against. Much better than what it was at release but never good.

so lets think about the Legion, a bunch of angsty teens Frank, Julie, Susie and Joey. that got pulled into the realm by the entity after they collaborated in killing a janitor.

Sure... not super grand but lets work with this.

We are Legion.
You start each match as Frank. with Susie, Julie and Joey scattered about the map near the edges.
By pressing the secondary power button you can choose to posses one of the other members. causing the controlled member to stop being unable to move and go through an animation for 2 seconds. then gaining the undetectable effect for 1.5 seconds.

Uncontrolled members have no terror radius.
Uncontrolled members will default to leaning next to an Exterior wall, loop wall, or other terrain (in that order of preference)

We are Legion has a 90 seconds cooldown.

Feral Frenzy
By holding down the power button charge up for a sprint. upon release sprint in a direction with Moderate turning. while sprinting you can vault windows and pallets and attack. On an attack hit stab repeatedly (able to hit multiple survivors.) dealing one state of damage and applying Deep wounds. Additionally refresh We are Legion.

On a Miss the Legion is stunned briefly.

Now this change fixes two things one Feral Frenzy isn't just a loop win, or if the survivor is already injured, useless. And the Legion now has impressive map pressure but on a significant cooldown. However the benefits don't end there.

I bought an ocarina from the Dark Web. It changed my life forever.

I guess you could trace my current situation back to two factors. My new year’s resolution and my weakness to aggressive marketing.

It started this Christmas, that I returned home after a long time. It was nice to see my family after being abroad and constantly on the road, working. Unfortunately, my employment makes me travel a lot. You see I am a public relations manager for a big company, and this forces me to be in all kinds of different places, all over the world. Of course, I don’t complain, and travelling is always a beautiful experience, but you can get homesick pretty easily.

Now, having travelled pretty much everywhere this year, I managed to attend all sorts of events, mainly music concerts from bands that very rarely visit my homeplace. From Muse, to Dream Theater, to my childhood love which was Manowar, and then back to rock Music with Sum 41.

I am sharing these moments because they were what flared up my old passion. The love I used to have for playing music myself.

After spending the first few holiday nights at my parent’s place, I got to their basement and rediscovered my old electric guitar and my amplifier, that I had almost forgotten there, for more than 6 years. You can imagine that the playing was very clumsy, but I managed to pull out some chords and basic melodies.

My job had made me forget about music, but now that I found my passion again, I vowed to not lose it again. It was honestly a wholesome moment when I managed to relearn some old songs and play them, while singing out with the family like we did back when I was just a kid.

Unfortunately, reality hits you no matter what you do. While preparing for my flight home, I did pack my guitar and a small practice amp I had just bought, but I knew I wouldn’t play with them that much. Next week I would have to travel again, and my schedule would be really busy, so realistically I would very rarely, even touch the guitar.

Nevertheless, I had started looking for some guitar gear, and since google is smarter than me, while I was doing my research, it started bombarding me with adds about small portable instruments, that you could easily learn. Normally I just ditch such kinds of ads, but this time they looked interesting. A small instrument that didn’t need extensive practicing and that I could play with any time? This actually sounded practical and interesting. And the prices weren’t too shabby either.

DEMOLITION DAYS, PART 35

Continuing

The next morning, everyone was itching to go prospecting. Our final destination was some 350 kilometers distant in the Flaming Cliffs, but we needed to get the lay of the land. We decided it was best to start out here where things were moderately friendly and put our scientific mettle to the test rather than risk it all at our final destination.

Finds of importance were almost immediate. We found several very well preserved dinosaur skeletons; it seemed almost too easy, here in this target-rich environment. We weren’t going to collect these, but rather mapped, flagged and preserved them for later recovery.

Es found a clutch of dinosaur eggs just a few meters from our campsite, around the back of a badlands-style hoodoo. It was entirely in situ, in place as the day it was laid. I took many arduous sedimentological cores for later evaluation. We were both proud and excited when the discovery was confirmed to be a clutch of Velociraptor mongoliensis eggs.

Carnivore eggs are quite rare though herbivore eggs and nests were found with regularity. I helped with the preparation and preserving in the field; as this one was going with us. Her discovery is now in the Mongolian Dinosaur Museum back in Ulaanbaatar.

There were more fossils of more diverse types than any of us have ever seen. There were early mammal fossils, multituberculate fossils, dinosaur material by the literal ton, crocodiles, turtles, fish, bird, and invertebrate remains.

We had found an ancient ephemeral lake which had dried up during a severe drought some 75 million years ago. An entire ecosystem had been preserved here and it still, to this day, being excavated and studied. Termed a thanatocoenosis,it was a death assemblage of the animals living, and dying, in the area. It was a colossal find.

Three days passed all too quickly, but in order to keep on our tight schedule, we had to leave the lakebeds and head further inland. South, towards China, into the very maw of the desert.

Onward to Bain-Dzak, the Flaming Cliffs of yore.

Day after day, we made stops and did some prospecting and updating of the geological maps. Some days, nothing was found. Other days, it was a cornucopia of fossils. It was an odd and unusual situation; but not until one of our nightly powwows did the Japanese crew make the suggestion that there wa

We were six

I got a travel nurse offer for a 12 week gig in a nearby small city. The job offered a housing stipend with corporate discount to an extended stay hotel. I thought about it. It would get me through these cold months, likely in a Marriott. But nah. I am at this weird point where I’d much rather be poor than the hook anywhere 40+ hrs/week. I’d rather rough it on my off days. To feel free and be free is more important than anything now. Restlessness and even transience is genetic I believe.

My father was a mechanic by trade. He was a wiry a guy and a chain smoker. He never held any one job for too long, but he returned to many. He was always welcome back and his exits were never dramatic. It was intuitive that he was somebody who never stayed in one place. He used to go to Milan Michigan every year, where he worked in some type of specialty shop on the drag racing cars. He said he was staying with his brother, but it wasn’t really his brother because to my knowledge he had no brothers whom he was in touch with. He did grow up in an orphanage in Indiana, where he also worked as a farm helper for Quaker family. This was apparently a good time in his life because he always spoke positively of the Quakes, and I believe he was officially a Quaker himself during the Vietnam draft. Anyway, he always came back from his Michigan sojourns with a pocket full of cash and we lived well for a good few months.

When flush like this, we ate dinner every night at a cafeteria. These places don’t exist anymore. The closest thing now would be an all you can eat salad and soup bar at a chain restaurant. Except the cafeteria we went to usually had a choice of entrée (roast beef or chicken, for example). Then you went along the sneeze guard line and picked from what seemed like miles of sides and desserts. Green beans, coleslaw, glazed carrots, mashed potatoes, baked potatoes, iceberg lettuce salads in little styrofoam bowls, buttered peas, 2 soups, rolls, biscuits, gravy, a rainbow of jellos and so many different kinds of pie. This was always a big treat. I have a strange memory of my parents both walking in with open beer cans. I think this couldn’t possibly be correct, but maybe it is. My memory is that nobody cared. Then the cash would run dry and he would scramble around changing oil and fixing up beaters while my mother would have her sewing/alterations business in full swing. And the cycle would go on like this.

Percussive Maintenance

This is the story of how the federation truly learned about brawn.

The Terrans, or humans as they prefer to be called, are by no means stupid, most certainly not smart however. They are delightful to communicate with through our translators, and have a very interesting past and present within our federation. Many many cycles ago, the federation was formed among peaceful planets, large and small, to create a web of protection and trade that covers as far as FTL travel has taken us. Our common enemy being the Omniflesh that have almost as vast a network as the Federation. They have committed unspeakable evil across the vast expanse of space, and have shown no remorse, and no signs of slowing. We fight, battle, wage war, and have been successful at protecting many of our planets and solar systems, but they have also taken so many from us. They convert the survivors, pit our former allies against us, resource ravage planets, and leave it a husk of floating despair.

What's important to know about my species, as well as every other species we've come in contact with including the Omniflesh, is that we all share similar primordial soup. We're not all the same, but our evolutions all took a very similar path, even spread across the cosmos. Our brains developed at an exponential rate after the discovery of Nextorium. This incredibly complex chemical chain is present in 99.99% of star systems, and it specifically targets brain growth, in both size and synapses. It's a joy to observe a primitive planet discover this, the change is so rapid and immense, within a few cycles they are already capable of joining the federation and become valuable assets.

There is one system, out of sheer cosmic randomness, that is absent of Nextorium. For thousands of cycles we've watched them flail about their planet, in a miserable pit of disease, civil war, and planetary destruction. They are large creatures, although they aren't much taller than myself or my mate, their minuscule heads look absolutely ridiculous perched upon a massive frame. Broad shoulders wider than my head, two arms and two legs, each individually thicker than my whole body. It took them so long to make contact we had little hope of them being useful additions, but who are we to turn away willing allies? It was a slow and arduous task integrating them into our fleet, but even with their history of violence and mental sluggishness, they proved to be excellent transporters. A Song by Song Review of Infest the Rat's Nest

Disclaimer: I am not a musician, and know very little about the technical aspects of making music. So bear with me if I repeat myself or don't go into too much depth throughout this review. I also am not a big metal fan so it's hard for me to compare this album to other music from the genre. I just love this band and music in general and wanted to give my opinion on the album.

I've been anticipating this album more than I have any album in a good while. When Self Immolate came out, I listened to it daily and it got me obsessed with this amazing band. I'm not a metal fan but any means, but the ability of this band to make a genre I'm not incredibly familiar with so enjoyable is astounding. So here's what I think about this album so far.

Planet B:

I wasn't enthralled with this song when I first started listening to it, but I heavily turn the volume up when it comes on now. The drums in the beginning are fast and feel hectic, but find their fit once the song gets going. The guitar riffs are insanely heavy and twisted and when I feel like they're at their climax they continue to grow. The vocal execution on this song, and many more on this album, are what make it amazing though. The emotion translates to me and makes the song feel like more than just a piece of music. I wish I could play guitar so I could rip away like they do on this song.

Story: The song starts off the album with the sentiment that our world is dying, and there will not be another for us to live on once it's gone. Heavily environmentalist but dark and twisted at the same time. We have done this to ourselves, and now we will suffer the consequences of our actions, and all of those who came before us. Is it fair? Yes and no. But life is flawed and unfair and this album is all about explaining this in some of the most rough ways imaginable.

Rating: 9.0/10

Mars For the Rich:

Unlike Planet B, this one hit with me near instantly. The guitar feels even heavier and executed so well even though the song does feel a bit less metal like than most on this album. Again, the vocals are filled with such rage and darkness that I furrow my brows and shout along to the entire song. They are deep and echo what feels like the truly primal feelings of humans. I don't know exactly what to call it, but the siren sound in the background of the verses is something I haven't heard in music many times and creates such an unexplainable atmosphere. The guitar riff that accompanies it fits

Need medical help. I'm about to close the deal with a lovely girl I'm dating and I can't stop farting acid demons.

Before heading to her place I dipped into the HaHa cafe comedy club to rip a couple planks and with them some gas. I thought this was a great sign and that the gas could have only been associated with whatever meal those nuggets were. Laced with broccoli my guess.

But as I was washing my hands I saw how bloated my guts were, big ol' gas bubble. Dead give away another low pressure system was making its way to the launch pad.

I swung by Ralph’s to pick up some strawberries and Tangerlos… These fruits are un gas related it was really just not to show up empty handed because she just had a long day.

I arrive at her place… I called it a “backyard bungalow” …to explain what this is, in California homeowners outfit garages or backyard structures into studios. This is important to the story because these structures are very basic when it comes to ventilation. Her bathroom had NO FAN. Just a small window above the toilet. It was already cracked, there was no breeze, I could smell bleach so she already cleaned up the bathroom… I looked around the bathroom for anything that was a scented spray… Nothing, not even perfume. Could you imagine Chanel no5 in my ass? Or Chanel no5 farts? There was scented lotion but how is that gonna go? I wipe my ass with rose water crabtree and Evelyn hand lotion? What if it stings my asshole?

***Thought came to me*** You know those Poo-poorii sprays that you spray onto toilet water and it knocks out the smell? What if I spray a version of that either directly onto my asshole or onto the fabric of my briefs where my fart gas passes through? If the product could some way coat every airborne feces particle of my ass steam with a pleasant smell…. Well, problem solved.

But I had no spray and I don't know if such a thing exists. Would love to see that on shark tank.

To add some color to the story, this girl is a stunner 5’8 Hungarian brunette with a glowing smile. She has a very warm personality and can carry an intelligent conversation. She has a strong spiritual practice that I attribute to her levity, countenance, and big ol' smile. She is a Physical Therapist so she has these great strong arms hands shoulders.

She is the tantric type that likes a slow build, push to the edge and back off of it. I am all about this and have the control to play with the brakes and tease the climax.

Some thoughts on Awesome Wave

Hello everyone!

I've been listening to Alt-J for a number of years, but its only recently that I've started to fully appreciate the music. I've had them on near-repeat for almost the past couple months, which is something I can't say for many other musicians/groups. I wanted to take some time to write some thoughts about their music and try to nail down what exactly it is I like about it (disclaimer: I'm not much of a writer). I've decided to do a short piece on their first album, an Awesome Wave (2012).

Awesome Wave has 11 songs, not counting the interludes, and there's something to like about all of them. The album definitely has a fairly consistent sound, in terms of instrumentation, mood, and lyrics. They use a fairly loose song structure, in that they don't always strictly stick to a traditional verse-chorus format. Even when they do, they play with it heavily. For example, the vocal meter may be similar, but the harmony will have radically progressed. It is fairly common for them to have two distinct verses followed by an extended outro. Examples of this include Dissolve Me, Matilda, Taro, and Ms. The first verse of Ms is backed by a light ringing and some distant vocals. They're more concerned with the entire piece sounding good rather than fitting into a rigid pattern. 'Something Good' is a nice example of this. The lyric 'structure' would loosely be something like ABACBCA. In the first half of the song, the vocals are separated by keyboard arpeggios, but in the second half, these arpeggios blend into the vocals, as if letting the momentum of the song continue to build up. 'Fitzpleasure', what I consider the most unique song on the album, doesn't really have any structure, except for the repetition of an extremely unsettling lyric, one that is consistent with the general frantic tone of the song. The lyrics are a reference to a disturbing gang-rape scene from Hubert Selby's novel Last Exit to Brooklyn. Coronavirus is making me depressed

I know I will sound so selfish as people are dying from this disease but I can't help but notice my mental health is slowly deteriorating.

I am already a germophobe due to my OCD (medically diagnosed) but the constant need not want to clean every surface if I so much as even set a cup of coffee down is driving me nuts.

I am unable to see my long term boyfriend of 8 years, I have no social life, I am college but I am going to end up failing as I am left to my own accord to do my work at home without a timetable or routine and I just feel like what is even the point anymore?

I am someone with underlaying problems as I have asthma and autoimmune disease so I am at risk , I was fine dealing with 2 weeks in isolation but now I am being told it is months of isolation and even after that the disease will still be around for 3 years, I am going crazy in my house.

I was fine with being in a house with my family because I am an introvert but never in my life have I ever wanted to go out and socialise before, it's the thought of not having the option of whether to stay in or not but now I HAVE to stay in.

I know I am going to butt heads with my family as I tend to keep my distance from them and keep myself busy cause all we do is fight most of the time and I am not stuck here for the foreseeable future.

I look after the planet, I am a vegetarian, I care about the world and I did every percussion I could to have a healthy and clean life and now everything is a mess. I hope this makes people realise how all of our bad habits have led until now it wasn't just the Chinese, it was global warming, sea and air pollution, casual meetups on tinder, it all contributed to this. All I want is for life to go back to normal but it never will as this is a wake-up call that we need to do better, this is proof that we are killing the one thing that has everything we want; the earth.

The lazy rework of the Legion

I've brought this up a few times but the Legions rework is painfully lazy. Its extremely disappointing to me that the devs nerfed my favorite killer so hard and gave them nothing to compensate for it. The Devs heavily changed the legion without changing most of their add-ons which really fucked up the killer. I know its probably not the highest priority with the new Wake up, BBQ n Chili, Solidarity, and several other bugs brought in with this update but since Double Blade Legion is back I was hoping the devs could actually put some effort into legion while they're fixing these bugs.

Legions Add-ons Legions add-ons are the main reason I'm posting this. The devs reworked legion to the point where their entire ability was pretty much scrapped. Yet, for reasons only the devs will ever know they didn't bother to make an overwhelming majority of the add-ons work with the power. Rendering them completely useless. Lets review.

  • Fuming Mix Tape Now this one is the biggest surprise. As people have been telling the devs since Legions first debut that this add-ons was a terrible idea and made no sense for the killer. This add-on is a ripoff of Wraiths very own "All Seeing" - Spirit add-on. All Seeing makes sense for Wraith. Wraith spends a lot of time in the Spirit realm as it benefits him. He moves faster so he uses it for transportation and he decreases his chances of being seen across the map, can sneak up on people, and puts pressure and worry on survivors. So whats better then an add-on that can tell you where the survivors are working at? However this is NOT the case for Legion. Legions ability is rarely used for transportation. It is an offensive ability. Furthermore, Legion cannot erase their heartbeat. And the excuse that perks like tinkerer can do that is irrelevant as powers should not rely on a perk and neither should ultra rare add-ons. ESPECIALLY ultra rare add-ons. Because of this uncontrolable large heart beat, it makes absolutely no sense that the Legion would have an add-on like this as they cannot use it to sneak up on survivors. Now one might argue, "the Legion can pop into frenzy to look at the auras quickly." or "the Legion can look while it a Frenzy chase to see where to head to next." but to that I argue that the Legions ability is no doubt an offensive one. Legion players should not have to look at the survivor, focus on where they are going and if they are going to attempt to outplay their swing AND try to look at the gens. And Legion

Type me I guess?

Going to be following the guide (don't trust my own descriptions of myself).

  • Age. Gender. General description
  1. Female. Don't think there's much to put here that I wouldn't add with the rest of the questions but it doesn't hurt I guess to do this here. Um, so I would say I'm mature for my age, quiet the majority of the time (will change based of my environment and my relationship with the people around me. Around school and peers I'm quite quiet. Around friends I can kinda be a jokester if I'm in a positive mood (rare). I like making people feel good. Kinda of a therapist to people, often comes at the price of my own emotional well-being. Probably be a therapist to more people if I actually got around to talking to new people, or having a second conversation with people but that's not a me kinda thing. I'm serious about things most of the time, but I'll have weird moods sometimes where I become extremely careless and unaware of consequences. I'm pretty average academically, would Love to be more than average but that's a tough for me to get around to I guess (procrastination and demotivation). Music is nice, too. I would say that's kinda it?
  • Medical diagnosis?

No, doctors and psychiatrists are scary.

  • Upbringing. Influence, response

I'm in a pretty religious and strict family (in my opinion, probably exaggerated). My dad is pretty overbearing for me. We've gone head to head a lot and I don't really get any sorta backup even when I'm right. My dad is pretty harsh and pays zero attention to emotion, gets all like "suck it up princess" about everything. It's annoying. It's pushed me into isolating and recently rebelling a bit. Parents get upset at the isolating so I kinda break down a lot emotionally with all the "constructive criticism" I receive. Still very religious, fell away for a little while, but a very important person to me, brought me back on track a while back. Also have a younger sibling (4 and a half year difference), she's the complete opposite of me and it's frustrating.

  • Career

I have no career. All state some areas that I would be interested in pursuing. Psychology, law enforcement, literally anything music related. Current plan is to try and become a marriage counselor, if I could work on making and producing music on the side, that would be nice.

  • Spending a entire weekend by yourself

Nursing 101: Caring for your loved ones at home

Your goal as a home healthcare provider is to build and support the immune system, limit the progression of the disease, and effectively communicate with local healthcare providers to determine if additional care or hospitalization is needed.

Start a medical folder or chart for each person while everyone is still healthy if possible. Have this on hand during phone calls with medical providers and for medical care/hospitalizations.

  • Birthday
  • Height and Weight
  • Medical history: diagnoses, surgeries, hospitalizations
  • Medications-prescription and herbal supplements: name, dose, frequency. For as needed medications like Tylenol or Tums, note the date and time taken.
  • Normal vital signs: Pulse beats per minute. Temperature. Respirations per minute. Blood Oxygen Level % (pulse oximeters can be purchased online and in drug stores inexpensively). Pain level on a scale of 0-10 including: location, duration, description. Tracking for seven to ten days at different times should be sufficient to establish a baseline. *Blood pressure readings at home can be inaccurate and cause alarm so unless you have a loved one with bp issues, I might skip bp. To double check an at home bp reading, you can visit your local pharmacy. You may also want to purchase a stethoscope and take an online crash course on listening to the various breath sounds.
  • Include: a chart with normal vital signs by age group, charts for proper Tylenol and Ibuprofen doses by age/weight. These can be given at the same time. Tylenol every 4 hours, Ibuprofen every 6. Some doctors recommend alternating every 3 hours to have continuous coverage.
  • Copy of ID and insurance card if you have one.
  • Research your medical system/insurance. Find out who your primary care doctor is, if your insurance covers telephone nurses or doctors, in home visits, urgent care centers, and emergency rooms. Keep these names, numbers, and locations in each folder.

When illness begins:

  • Note onset date, symptoms, and vital signs every 4-8 hours depending on severity.
  • Note calls to physicians and their advice.
  • Note interventions. As symptoms become more severe also document vital signs before and after to see if what you are doing is working.

Caring for someone with viral respiratory symptoms:

  • Wash your hands with soap and water before and after each contact. Wear a protective mask if available.

Interesting case I evaluated today

Very interesting case I evaluated this afternoon. I think this would be valuable for students to problem solve through as they read it over. Were you headed in the right direction?

44yo male presents with abdominal and LBP L>R. States that this past Wednesday he was helping his company deliver and doing a lot of lifting he doesn’t do because he usually works in the warehouse. His pain however began last night, not immediately after the injury or even 24 hours later. It’s primarily aggravated by moving around in bed at night. He also states that for the past 2-3 weeks his back has been hurting when he sits but he doesn’t know if they are related. Denies LE symptoms, numbness or tingling. No history of LBP.

States that for the last 2 days he really hasn’t been moving, eating, drinking or doing much of anything other than work or sleep. He thought he was spiking a temp but his wife checked it last night and it was normal. He also noticed that he has been urinating a lot despite not drinking that much water and that his urine is dark, no trouble initiating a stream and no pain urinating. No bowel issues.

Objective:

Lumbar ROM WNL pain free in all directions

Via squat screen B LE ROM pain free WNL

B hip ROM in supine WNL pain free

No TTP across pelvis, Sacrum or lumbar spine

Strength B LE and hips WNL

TTP Left lower quadrant (LLQ)

Percussion to LLQ painful

Rebound tenderness LLQ painful

Hard mass felt in LLQ

Thoughts?

I referred him to the ER with concerns for kidney issues. Definitely non mechanical LBP.

The fucked up part? I work in a workers comp OP setting so he was evaluated by a physician yesterday (literally less than 24h ago) and subsequently referred to me for LBP. I feel like a lot of people in this field forget that we really are medical professionals.

I'm the captain of an oil rig in the North Sea. We found something. [Part 1]

The divers boiled out of the diving bell, a wet mess of inky neoprene and rubber tubing. They looked like a mass of black snakes.

Oleg tore off his regulator and ripped the helmet from his head. It clanged on the steel of the diving platform, shattered faceplate glittering in the sodium light.

“Jesus fuck, Yevgeni – it got Alexei! It tore him…” He paused and threw up a mass of foam and blood.

I knelt down beside him, cradled his head.

“You didn’t decompress properly, Olya.” I could see bubbles of nitrogen moving through his skin. His scalp felt like bubble wrap as I stroked it. I knew I didn’t have long. He didn’t have long.

I turned his swollen face to me, locked his eyes with mine.

“What did you see, Oleg? Is the rig in danger?”

He wiped a trail of mucus and blood from his mouth and spat pink, foamy gobs onto the platform.

“It came out of nowhere, Zhenya – nowhere. And it was huge, captain! Massive! It – it…” he doubled over and clutched his midsection. The blue water in the bottom of the diving bell bay boiled and bubbled, streaks of pink shot through it as the divers coughed and vomited their ruined lungs into the swirling water.

They all died, one after another, within minutes, their bodies bent and broken by the pressure and nitrogen bubbling up in their blood. The whole dive team of 5 – minus Alexei, still out there in the deep - 600 meters below the wine-dark sea.

I brought up the topographical map of the rig site on the monitor. It glowed green in the darkened mess. The 6 of us remaining on the rig gathered around it like Macbeth’s witches around the cauldron.

“We lost contact with the deep-water team here -” I said, tapping the screen at a point labeled Dunsinane Abyssal.

Bioengineering has created a human variant which ages and develops from a newborn to an adult human in just three days. One such being is raised as a soldier from birth, but slowly realizes that everything she’s been raised to believe is a lie.

I remember being born.

I don’t remember all of it—the quiet, safe slumber inside the mechanical womb, the towels and fussing, the injections and implants—those just blurred together, although I’m sure the process was extensively documented by the military scientists present at the birth of my siblings and I. But despite what was lost to the mists of time, I still remember the highlights.

I am human, after all.

When I was born, I cried exactly once. Less of a cry, really, and more of a mewl. It was hard to keep crying, though, when I was surrounded with so many wonders.

Lines of babies extended to either side of me, for as far as the newborn eye could see. I think my senses were still a bit muddled up—they looked like the smell of a hospital, and their cries tasted like lemonade.

A sound caught my attention—the child to my left was burbling. I listened for a few heartbeats, then gurgled back:

“Glblglb?”

The other child stared at me, surprised, then smiled, hands waving in the air. “Dwee-bull! Ybblptth sllmpbbl.”

I nodded sagely. Wise words, for one so young—for they were words, indeed. Words I hadn’t deciphered yet, but words which held a meaning nonetheless. I tapped myself on the cheek and said, “Eurybmph bloblahcha. Ybbltth blo—”

“My God, are they communicating?” Alas, my deeply stimulating intellectual conversation was cut off by a scientist’s exclamation. Every baby in the room stared at her. “Alright, we've seen what these things can do. We should separate them for now—the state doesn’t need another Young Rebellion on our hands.”

“We can’t move them yet—we’d have to give them nutrition packs of their own.” A man in a dark, crisp uniform pointed behind us; instinctively, we all turned as one. Indeed, strange tubes holding a dark, sludgy substance led into our bodies. “State protocols mandate that they be kept together for the first 12 hours of development anyway; it’s easier to get them up to speed.”

At that, the woman frowned. “We could at least erect some barriers between them. I don’t like how they’re all staring. Shouldn’t babies cry?”

Danish chief medical doctor says; “wearing a mask is ridicules” and “a drastic measure”

Danish state news - DR, brought a Q&A with a chief medical doctor at Hvidovre Hospital, Gitte Kronborg, to answer the general question from the public such as; how dangerous is the virus actually - how sick are the people sick with corona really and what should one look out for. Here’s a few of the questions and the doctors reply;

Starting with the headliner

Q; my father is in a panic (mostly because he only reads the media’s headliners) and says what all his children (us) should wear a mask. I think it sounds ridiculous, what do you think?

A: I would I agree with you, it sounds ridiculous. Corona virus is only contagious in close contact and not contagious by you walking around in daily life . (My comment: this woman clearly does not take the metro in rush hour in Copenhagen!)

Q: Would you travel to Asia/Vietnam if you are healthy/ or have asthma.

A: Yes.

Q: So far the reports have said that only elderly people with a weak immune system was in the highest danger of dying. Is this changed by the fact that a 44 year old has died in the Philippines?

A: No.

Q: Is it a good idea to wear a mask that covers your nose and mouth when you are at the airport or in the airplane, or is too drastic?

A: It’s too drastic.

Q: Why should we be worried about coronavirus when a regular influenza killed 1500 Danes in 17/18?

A: We shouldn’t be worried. We do take new viruses seriously and try and learn from it. The country around us all take different percussion measures and we try to contribute to some reasonable precautions.

Q: How high is the risk to be infected with corona in Denmark?

A: The chance of being infected in Denmark at this time is low. All indications are that the disease is not quite as serious as first feared. Denmark has a well-functioning healthcare system. So there is no need to be afraid. (My comment; Hospitals across the country have a shortage of nurses so much so that regular surgeries are being canceled. The waiting lists for a surgery can be long, and nurses in the big cities are often overworked)

I scored each song with a similar single smack down chart, 10 points each for 3 categories, lyrics, vocals and instrumentation. Please remember, my list is not the right list, everyone has their own perceptions of music and I respect everyone's opinion on a song, even if it is something I truly don't like, so here is my top 20.

20. Catch-Brett Young

I need to get my guilty pleasure of the way.

This song is good, I really like it, but it is not top tier material. The pop vibes are pretty obvious but vocally this has a nice sound. He has a nice range, and while simplistic in terms of lyrics I really like how he uses the word catch in a lot of the song, you get a image in your head with the song. But I think this song has a really nice compromise and is one of his better singles off his latest album which is a whole lot of vanilla.

19. We Were-Keith Urban

This is what you get with a fantastic Eric Church write. This song may not be that good instrumentally but the lyrics are something I really like the lyrics. Vocally this song tends to lack anything truly interesting. But I love the imagery in this song and the change in the chorus. I just wish this song was sung by Eric Church fully because Keith doesn't bring much to the table.

18. Sixteen-Thomas Rhett

This song has a really nice progression, when he's 15 longing to be 16, to wanting to be 18 when he's 16 and we end with him reflecting on his life, how he his happy now and how he was when he was 16. This is a polished pop country song but I think it sets itself apart from the rest because of its story telling. I don't like the snap tracks because this could really use real percussion and the pop sounds don't really mesh well with the story he's trying to tell.

17. I Wish Grandpas Never Died-Riley Green

Okay before you say "invalid list" hear me out. This song is good, but it lacks anything truly outstanding to be higher on this list.Because if you think about it, what does this song truly offer that is outstanding? Maybe the sound of the song which has a nice Neo-traditional vibe but vocally, there is not much
